Why Medicare Advantage Is Booming in California

California leads the nation in Medicare Advantage enrollment, with over 3 million beneficiaries — nearly half the state’s Medicare population — choosing MA over Original Medicare. The appeal is straightforward: lower premiums, built-in drug coverage, and extras like dental and vision that Original Medicare doesn’t include.

But California’s MA market is also one of the most complex. With dozens of plans per county and significant variation in networks, getting the right plan matters.

How Medicare Advantage Works

When you join an MA plan, a private insurer approved by Medicare takes over your coverage. Your plan must cover everything Parts A and B cover, but it delivers that coverage through its own provider network, cost-sharing structure, and often adds benefits Original Medicare can’t match.

You still pay your Part B premium ($202.90/month in 2026), and many California MA plans charge $0 on top of that.

Plan Types Available Across California

  • HMOs — The most common type in California. You pick a primary care physician, need referrals for specialists, and must stay in-network (except emergencies). Typically the lowest premiums and copays.
  • PPOs — More flexibility to see out-of-network providers, though at higher cost. No referrals needed. Popular in metro areas like the Bay Area and LA where people see specialists across county lines.
  • Special Needs Plans (SNPs) — Tailored for Californians with specific conditions (chronic illness SNPs), institutional care needs, or dual eligibility for Medicare and Medi-Cal. California has one of the largest D-SNP markets in the country.

Extra Benefits California MA Plans Commonly Offer

These vary by plan and county, but in competitive California markets you’ll often find:

  • Dental coverage: Cleanings, X-rays, fillings, and sometimes major work like crowns — not covered by Original Medicare at all
  • Vision benefits: Annual eye exams and an allowance for glasses or contacts
  • Hearing: Annual hearing tests and hearing aid allowances (some plans cover up to $3,000+)
  • Over-the-counter allowance: A monthly card for OTC medications, vitamins, and health products
  • Transportation: Rides to medical appointments — especially valuable in spread-out California counties
  • Fitness programs: SilverSneakers, Silver&Fit, or Renew Active gym memberships
  • Meal delivery: Meals after hospital discharge or for those with chronic conditions

The Annual Out-of-Pocket Maximum

Every MA plan must cap your annual out-of-pocket spending. This is arguably the biggest advantage over Original Medicare, which has no spending cap. The federal in-network maximum for 2026 is $9,250 (down from $9,350 in 2025), and many California plans set limits well below that — often between $3,500 and $7,550.

Choosing the Right Plan in Your County

California’s 58 counties each have different plan options. A $0-premium HMO with great benefits in Los Angeles County may not be available across the county line in Ventura. When evaluating plans:

  • Check your doctors first. Verify that your primary care physician, specialists, and preferred hospital are in-network before anything else.
  • Run your prescriptions. Enter every medication into the plan’s formulary checker. A plan with a $0 premium but expensive drug copays may cost more overall.
  • Look at total annual cost — not just the premium. Add up premium + expected copays + drug costs + deductible for a realistic number.
  • Check star ratings. CMS rates every plan 1-5 stars. Plans with 4+ stars tend to offer better care coordination and fewer surprise denials.

Enrollment Windows

You can join or switch Medicare Advantage plans during three periods:

  • Your Initial Enrollment Period — the 7-month window around your 65th birthday
  • Annual Enrollment (Oct 15 – Dec 7) — switch plans or return to Original Medicare
  • MA Open Enrollment (Jan 1 – Mar 31) — switch between MA plans or drop MA for Original Medicare

Qualifying events (moving to a new county, losing employer coverage, gaining Medi-Cal) may also trigger a Special Enrollment Period.
